Clare Stoker is a Specialist Nurse who co-ordinates our Orthopaedic Elderly Care Nurse Specialist Service based at the Newcastle General Hospital, which looks after some of our most vulnerable patients.
She gets involved in the care of all patients over the age of 70 who are admitted to the Orthopaedic Trauma Unit following a fall or other injury, as well as all patients admitted with a hip fracture, regardless of age. Elderly patients who have fallen are prone to breaking their hip and or thigh bones.
This specialist service enables you to be seen by a Consultant specialising in the care of older people, if and when you need it. The cause of your fall and any other health related conditions such as osteoporosis can be rapidly identified and treated by our Fracture Risk Assessment Service and you can be also be transferred to orthopaedic rehabilitation for further care if required.
Sister Stoker liaises closely with colleagues in the community when arranging discharge to ensure that you receive further support at home if and when you need it.
