Pharmacy Services

Anticoagulant service (INR Clinic)

Managed by a specialist pharmacy team, the Out-patient Anticoagulant Service provides a monitoring service for patients on warfarin and other oral anticoagulants. 

Clinics are held, by appointment, in the Outpatient department of both Freeman Hospital and RVI sites as follows:-

Freeman (until 14th June 2018)

Monday to Friday (9am - 11:30am)

Freeman (from 14th June 2018)

Monday, Tuesday, Wednesday and Friday mornings (9am - 11:30am)

RVI

Monday, Thursday and Friday mornings (patients are allocated timed appointments).

All patients must be referred by a Trust Clinician, local GP or other NHS Trust using the Northern Network Anticoagulant Referral form.

The Community INR (warfarin) Service, which operates through some of the local GP surgeries, is managed by the Freeman Hospital Haematology Department and is not part of the Directorate of Pharmacy and Medicines Management. They can be contacted on the number below.
