Anticoagulant service (INR Clinic)
Managed by a specialist pharmacy team, the Out-patient Anticoagulant Service provides a monitoring service for patients on warfarin and other oral anticoagulants.
Clinics are held, by appointment, at the Freeman Hospital each morning (Monday to Friday) where patients may attend between 9:00 - 11:30am; and at the RVI's New Victoria Wing Outpatient Department on Monday, Tuesday and Friday mornings where patients are allocated timed appointments.
All patients must be referred by a Trust Clinician, local GP or other NHS Trust using the Northern Network Anticoagulant Referral form.

The Community INR (warfarin) Service, which operates through some of the local GP surgeries, is managed by the Freeman Hospital Haematology Department and is not part of the Directorate of Pharmacy and Medicines Management. They can be contacted on the number below.
