Managed by a specialist pharmacy team, the Out-patient Anticoagulant Service provides a monitoring service for patients on warfarin and other oral anticoagulants.
Clinics are held, by appointment, at the Freeman Hospital each morning (Monday to Friday) where patients may attend between 9:00 - 11:30am; and at the RVI's New Victoria Wing Outpatient Department on Monday, Tuesday and Friday mornings where patients are allocated timed appointments.
All patients must be referred by a Trust Clinician, local GP or other NHS Trust using the Northern Network Anticoagulant Referral form.
