Clinical Pharmacy Service

With the ongoing importance of drug safety issues and continuing emphasis on cost-effective prescribing within the NHS, a strong clinical pharmacy presence on the ward is essential to ensure the use of medication is both safe and appropriate, and that patients receive the best possible care.

Our Clinical Pharmacy Service operates at ward level across a wide range of areas on all three sites. Clinical pharmacy teams are based on the wards, providing services that include medication history taking and drug chart review, patient counselling, ordering of medication for patients’ treatment during their hospital stay, attending consultant ward rounds, and preparation of discharge prescriptions. They are also a useful source of information for the nursing and medical staff on the ward, actively offering advice and guidance on medication issues to promote the safe and effective use of medicines.

The service is headed by the Assistant Director of Pharmacy, Clinical Services and supported by an experienced team of Lead and Senior Clinical Pharmacists, each managing the provision of clinical pharmacy services within their individual areas. Services are provided at ward level by rotational pharmacists and clinical pharmacy technicians with lead and senior pharmacist support.

Information for patients

A ‘One-stop Dispensing’ service is available for many of the wards across the Trust, as part of our commitment to making better use of NHS resources - minimising medication wastage and significantly reducing waiting times for discharge prescriptions. This service involves the assessment and utilisation of patients’ own medication during their hospital stay and labelling all medication ready for discharge.

Once a discharge prescription is written, our clinical pharmacy team will check the available items against the prescription, arrange for any extra items to be dispensed, and make the discharge medications available on the ward ready for the patient’s discharge from hospital. 

During the patient’s hospital admission drugs are stored in the bedside locker, with nursing staff administering them at the appropriate time. On some wards there may be the option for patients to self-medicate.
